Designed to know as little as possible about you.

  • No name, email, phone number, or ID is ever asked for or stored.
  • Staff see only group trends — never individual responses.
  • If fewer than 10 teens submit in a group, their data is hidden entirely (k≥10 rule) to prevent individual identification.
  • There's no safety-screening question in the survey — every teen sees the same safety resources (a crisis line, a trusted adult, community support) on their results screen automatically, no matter how they answered.

Access to programs isn't always enough.

Many teens have access to community spaces but still struggle silently with stress, isolation, and school pressure. RiseWell creates a low-pressure, anonymous check-in — not a diagnosis, not therapy — that connects them to the right supports at the right time.
