How it works

Three steps to finding support.

Step 01
Check in anonymously
Answer 10 short questions about how you've been feeling lately. No name, email, or ID required. Takes about 1–2 minutes.

Step 02
See your support areas
The app maps your responses to broad support categories — like Stress Support or Social Connection — using clear, rule-based logic. No AI makes health decisions.

Step 03
Connect with resources
You'll see a small set of relevant community resources or self-guided supports — framed as "things that may help," never as a diagnosis.
Real community. Real support.

A place where young people thrive.

Community centers are where connection happens — and RiseWell helps staff meet teens where they are.

A Community Behind You
Our privacy promise

Designed to know as little as possible about you.

  • No name, email, phone number, or ID is ever asked for or stored.
  • Staff see only group trends — never individual responses.
  • If fewer than 10 teens submit in a group, their data is hidden entirely (k≥10 rule) to prevent individual identification.
  • The safety question is answered by choosing from fixed options — there's no free text to type. Your answer is processed only on this device and is never sent anywhere; if it suggests you might be in danger, supportive resources appear right there.

About RiseWell

Access to programs isn't always enough.

Many teens have access to community spaces but still struggle silently with stress, isolation, and school pressure. RiseWell creates a low-pressure, anonymous check-in — not a diagnosis, not therapy — that connects them to the right supports at the right time.

Built through Rise 'N' Reach, this tool is designed for the teens staff are already trying to reach — the ones who need a gentle on-ramp to support before they'll ask for it directly.
